Why does accounting disagree with operations?

Accounting and operations measure the same organization on different clocks and definitions. Operations runs on daily, real-time events: residents in care, staff on shift. Accounting runs on periods, billing rules and accruals. Adjustments, bed holds, payer rules and late documentation all separate the two. The disagreement is expected. Leaving it unreconciled is what causes problems.
Why this happens
Each team is accurate within its own discipline, so each defends its number, and leadership meetings become arguments about definitions.
Shared definitions and continuous reconciliation let both teams keep their discipline while agreeing on what governs.

The governance question
Finance needs to know not only the number but its authority: which source it came from, which definition applies, what was adjusted, and who approved the adjustment. That is provenance, and it should be inspectable before the number is used.
