What audit trail should exist behind executive KPIs?

Behind each executive KPI there should be a record of the source values, the definition and calculation version, every adjustment and override with its approver and reason, each reconciliation determination, and the refresh history. The trail should be complete enough for someone outside the team to reproduce the KPI without asking the person who built it.

Audit trails often record system changes but not business decisions. Both are needed.

Being able to reproduce a KPI is the practical test that an audit trail is complete.

A defensible executive number has an approved definition, a known source authority, a recorded resolution of any disagreement, and lineage back to source. Disputed numbers should be shown as disputed, not smoothed.
