Operator data governance framework: the definition-and-authority register
An operator data governance framework is only as real as its register: one row per governed metric recording the decision it supports, the versioned definition, the authoritative source, the reconciliation rule, the approver and the lineage retained. Download the register, keep the illustrative occupancy row as a model, and complete the blank rows for your own census, labor and financial metrics.

The six steps
Decision → Definition → Authoritative source → Reconciliation rule → Approver → Lineage
01 · Decision
Column 1 — the decision the row supports.
Which occupancy figure goes into the weekly ownership report and the staffing plan for the coming week.
02 · Definition
Column 2 — the versioned definition.
Occupancy v4: occupied licensed units divided by available licensed units at 11:59 p.m. local time on the last day of the reporting week, versioned and approved.
03 · Authoritative source
Column 3 — the declared source, per field.
Property management system, for unit status.
04 · Reconciliation rule
Column 4 — the tolerance and the prevailing source.
When the property management move-out date and the clinical discharge date differ by more than 24 hours, defer to the clinical discharge date.
05 · Approver
Column 5 — the accountable role and approval date.
A named accountable role (for example, the regional VP of operations), recorded with a timestamp.
06 · Lineage
Column 6 — what is retained and where.
All three readings (87.4%, 88.1%, 86.9%) retained unaltered, together with the definition version, rule and approval, so the governing value can be replayed later.
Illustrative example
One occupancy figure, three defensible readings
- Property management — Unit released on move-out day87.4% occupancy
- Clinical record — Unit held until discharge closed88.1% occupancy
- Billing calendar — Billed days, third calendar86.9% occupancy

Complete one row, then get it approved
A register earns authority one approved row at a time. Leave rows blank rather than guessing; an empty field is an open governance decision, not a gap to paper over.
