Move-in and the first 30 days, a change in condition, a transfer to the hospital, the care conference, the everyday texture of the day, and the complaint. Five of the six cross a departmental boundary — sales to clinical, clinical to business office, floor to administrator — and each boundary is where the promise made earlier stops being visible to the person keeping it now.
The regulated half nobody records as evidence
Under 42 CFR § 483.10 a resident has the right to be informed and to designate a representative who must be kept informed; § 483.10(g)(14) requires immediate notification on accident with injury, significant change in condition, altered treatment, or transfer and discharge; § 483.15 adds notice content, timing, and recipients including the state long-term care ombudsman. A notification recorded as prose cannot answer at what time, to whom, by what method, and with what acknowledgment. That mismatch is the exposure.

- No. Family communication apps handle the message. This pillar handles the record underneath the message: who the authorized contact is and under what relationship, what obligation-bearing notifications occurred and when, which questions were answered and which were re-asked, and how experience signals reconcile to the same resident and period. Operators keep the communication tools they already run.
- Why treat family engagement as a compliance surface rather than a satisfaction program?
- Because a material part of it already is one. Notification on a significant change in condition, on an accident with injury, and on transfer or discharge are regulated events under 42 CFR § 483.10 and § 483.15, with defined recipients and, for movement events, notice to the long-term care ombudsman. Satisfaction programming sits on top of that obligation; it does not replace it.
- What is a re-contact rate and why does it matter more than call volume?
- Re-contact rate is the share of family contacts that raise a topic already raised within a defined prior window. Call volume alone is ambiguous — it can rise because engagement improved or because nobody got an answer. Re-contact isolates the second case, and it is computable from contact events without any survey instrument.
- Do the calculator outputs represent results SeniorCRE has produced?

- How does HIPAA apply to family updates and photos?
- Disclosures to family members and personal representatives are governed by 45 CFR § 164.510(b) and § 164.502(g), and the content of any disclosure is scoped by the minimum necessary requirement. In practice this makes family engagement an identity, relationship, and authorization problem before it is a messaging problem.
- Can this replace our resident satisfaction survey?
- No, and it should not. Standardized instruments such as the AHRQ CAHPS nursing home family and resident surveys remain the methodologically strongest experience measurement available. The gap is that survey results arrive late and sit apart from complaint history, notification compliance, and participation data. The value here is reconciliation, not substitution.
