What makes an operating record governed?
Centralised data, attached evidence, human approval, change logs, AI permissions, rule-driven workflows and auditability are all important controls, and none of them alone makes a record governed. A record is governed when the operator has established seven things for each fact that matters: its definition, the source authorised for that definition, the decision the authority applies to, the reconciliation rule when another legitimate source differs, who adjudicates when the rule cannot, the lineage explaining why that value governed, and the preservation of the other valid facts that did not.
Why “governed” now needs a definition
Governance vocabulary has become common across senior housing & care software: governed records, systems of record, governance layers, human-attested determinations, evidence trails, institutional knowledge. Much of that work is substantive, and an operator should credit it rather than discount it.
It does mean the word no longer distinguishes anything on its own. The useful question is narrower: what does the governance have authority over? Governing a workflow end to end is not the same as governing which meaning the enterprise relies upon across workflows.
The seven objects a governed record requires
These are the objects the operator establishes. They describe what the DATA → TRUTH transition consists of; they are not a replacement for the four-stage chain.
Definition. What exactly the fact means, including the measurement moment.
Source authority. Which source is authorised for that definition, and why.
Decision context. For what decision that authority applies.
Reconciliation. What happens when another legitimate source differs.
Adjudication. Who possesses authority to resolve disagreement when policy cannot.
Lineage. Why this value and this definition governed.
Preservation. Which other valid facts existed and why they did not govern this decision.
Attestation and authority answer different questions
Attestation answers “who approved this determination?” Authority answers “why did this definition govern this decision?” Auditability proves what happened; authority lineage proves why it was allowed to govern.
Human review, human attestation and institutional authority are three concepts, not one. Review means a person checked the output. Attestation means a person took responsibility. Institutional authority means the organisation established, in advance, that this role decides what governs this definition or decision.
Worked example: two valid values, two decisions
Operational occupancy of 91.7% and revenue occupancy of 89.9% can both be valid. For capital planning the applicable definition is revenue occupancy, the applicable authority is the financial operating record, the governed value is 89.9%, and 91.7% is preserved as a competing valid fact. For staffing the applicable definition is operational occupancy and the governed value is 91.7%.
A governed record does not force one globally surviving answer. The operator establishes which valid meaning governs each consequential decision, and the authority rule and decision lineage are recorded. Figures here are illustrative and are not operator results.
What this means for AI
An AI system should receive the governed fact together with the applicable definition, the authority, the decision context, the conflict state, the lineage and the permitted use. Where the governance state is unresolved, the system should surface the conflict and escalate rather than silently choose which source becomes enterprise truth.
DATA → TRUTH → DECISION → EXECUTION. Data arrives from the systems that already hold it; truth is what the operator has ruled the business will accept; decision is the action taken on that ruling; execution is what actually happened, recorded against it.
What this answer does not assert
This answer describes an architecture and a governance method. No measured outcome at any operator is claimed.
