Do I need to replace my EHR to use AI?
No. Replacing the clinical system is a deployment choice, not a prerequisite for AI. The prerequisite is governance: before any model or agent is useful, the operator has to state what each number means, which system is authoritative for it, what happens when two authorized sources disagree, and who rules when the rule runs out. AI applied to ungoverned data produces confident answers no one can defend.
The coexistence path, stated plainly
Keep the systems you run today. The EHR keeps the chart, the scheduler keeps the shift, the ledger keeps the entry. What the operator adds above them is a governed operating record: the definition, the authorized source, the reconciliation rule and the recorded ruling for each fact the business acts on. AI then reads governed facts with their authority and lineage attached, not raw exports from three systems that count census at three different moments.

The governance prerequisite for AI
An AI system should receive the governed value, the definition it satisfies, the source that holds authority, the decision it may be used for, the conflict state and the permitted action. Where governance is unresolved, the correct behavior is to surface the disagreement and escalate, not to quietly pick a source and make it enterprise truth.
DATA → TRUTH → DECISION → EXECUTION. Data arrives from the systems that already hold it; truth is what the operator has ruled the business will accept; decision is the action taken on that ruling; execution is what actually happened, recorded against it.
Definition. One written meaning per number, including the moment of measurement.
Source authority. A named authoritative system per field, and the reason it holds authority.
Reconciliation. A deterministic rule for two authorized sources that disagree.
Authorization. What any model or agent may read, act on and escalate, declared before it runs.
When replacing a system is the better answer
Replacement earns its cost when the incumbent cannot produce the field at all, when its record cannot be read without manual re-entry, or when the operator wants clinical, labor, census and revenue on one model by choice. That is a migration decision with its own gates: source access, security review, clinical-safety configuration, parallel validation where required, and operator acceptance in writing.
Govern first. Replace only by choice.
